Records Request

Request a copy of a police report, incident report, accident report or other department record.

Use this form for records about an incident you were involved in or have a direct interest in — for example a report needed for an insurance claim. The more you can tell us (a case number, the date, names of those involved), the faster we can find the record.

Requests under the Illinois Freedom of Information Act are handled separately through the Village FOIA portal. Crash reports can also be obtained directly from the state crash report service.
