This form is exclusively for government agency personnel requesting records or information for official inter-agency purposes. A valid government email address (.gov) is required. Requests are processed in coordination with the submitting agency and may require additional verification. Supporting documentation, such as official request letters or court orders, can be uploaded with your submission.
